Unable to add cards or users.. "apply" greyed out.

I am hoping someone can provide some guidance. We had winpak 2005 running on an XP box. IT built a new pc running windows 7 and installed WinPAk 4.0 SE on it. I had to migrate the SQL database from the old version and have the system running on Windows 7. I can lock and unlock doors and all my cards and user names, holiday schedules etc. are in the database. If I try to add a new card number or a new user the apply button is greyed out and if I hit OK the screen closes normally and you are back at the original card window with a blank line added at the bottom like it is creating a blank field.

I am running SE 4 build 673.3 and the license is good.
