Notifire-NFS 640E Password forget

I have one panle of notifire model No: NFS 640E.I forgat the supervisor possword.how to recover that.
The good news is it can be recovered, the bad news is it is a pain.
download the attached "notifier password agreement" docs. READ THEM
The short of it is, the customer needs to full it out, get it notarized, send it in and wait for a phone call from Notifier with the password.
